1. Program Overview
The CPAP Supply Support Program (“Program”) allows Careica Health to coordinate the replacement of eligible CPAP supplies and, where permitted, submit claims directly to applicable public, federal, employer-sponsored, or other third-party benefit programs on your behalf.
This Program is intended to support continuity of CPAP therapy by assisting with supply replacement timing and administrative billing processes. Careica Health is not the insurer, benefit provider, or payer and does not determine coverage eligibility or reimbursement decisions.
2. Participation and Authorization
By participating in the CPAP Supply Support Program, you authorize Careica Health to:
- prepare eligible CPAP supply replacements in accordance with applicable replacement schedules and coverage rules
- submit claims to your applicable benefit program(s) on your behalf, where permitted
- contact you regarding upcoming replacements, coverage requirements, or administrative matters related to your CPAP supplies
Participation in this Program does not guarantee coverage, approval, or reimbursement by any benefit provider.
3. Supply Replacement Timing
CPAP supplies may be prepared for replacement based on:
- standard replacement guidelines
- eligibility and frequency rules of the applicable benefit program
- the information Careica Health has on file at the time of processing
Replacement timing may vary depending on benefit program rules, documentation requirements, clinical considerations, or changes to eligibility.
4. Patient Responsibilities
You are responsible for notifying Careica Health as soon as possible if:
- your insurance or benefit coverage changes
- you become covered under a different plan
- you no longer wish to receive a scheduled supply replacement
- your contact, delivery, or pickup information changes
Failure to notify Careica Health of changes may result in delays, denied claims, or cancellation of a supply replacement.
5. Opt-Out and Cancellation
Participation in the CPAP Supply Support Program is voluntary and may be withdrawn at any time.
You may:
- cancel a scheduled supply replacement, or
- opt out of the Program entirely,
by contacting Careica Health prior to the processing or shipment of supplies.
Opting out of the Program will not affect your ability to obtain CPAP supplies through other arrangements or payment methods.
6. No Personal Payment Authorization
The CPAP Supply Support Program does not involve:
- credit card storage
- automatic billing to a personal payment method
- membership fees or subscription charges
If payment is required for any reason (including ineligibility or partial coverage), Careica Health will contact you before proceeding.
7. Limitation of Responsibility
Careica Health is not responsible for:
- coverage or reimbursement decisions made by benefit providers
- claim denials related to eligibility, frequency limits, documentation requirements, or policy changes
- changes to benefit program rules or coverage criteria
